Replace a Lost/Stolen/Mutilated Passport
To replace a lost/stolen/mutilated passport, or a passport expired more than five years ago, you must appear personally at the Embassy in Bern or the Consular Agency in Zurich or Geneva. You do not need an appointment if applying in Bern or Geneva but must schedule an appointment in Zurich . (Please note: replacement passports will take longer to process at the Zurich or Geneva Consular Agencies. If you require a new passport as soon as possible, you should submit your application in person at the Embassy in Bern.) You should submit your application for a replacement passport as soon as possible, since it may not be possible to replace your passport within one working day.
At the time of your visit, you will be required to complete/present the following documentation:
- An application form for replacement passport, form DS-11
- A completed application form DS-64, Statement regarding lost or stolen passport
- Evidence of your United States citizenship: (Originals) e.g.:
- A U.S. birth certificate
- Naturalization Certificate
- Certificate of Citizenship
- Consular Report of Birth Abroad
- An expired U.S. Passport
- Identification: (with a photograph) e.g.:
- Driver's License
- Foreign passport
- Identity card
- Swiss Residence permit
All my ID's were stolen along with my passport. Now what do I do?
If all of your Identity documents were stolen along with your passport, bring someone with you (family member, traveling companion, or someone who knows you well) who is a US Citizen and who is willing to vouch for your citizenship and identity and can provide us with their original US passport.
Two color photographs. Passport photograph requirements are strictly enforced.
Applicants should be aware that the Embassy cannot accept any photographs not meeting these requirements, (i.e., photo too small, headsize too big, not identical, dark background or photo color copies).
Passport fee:
- $100.00 or CHF 110.00 (age 16 or older)
- $85.00 or CHF 93.50 (under age 16)
How can I pay?
The fee can be paid in cash in either currency. No personal checks are accepted. At the Embassy in Bern, traveler's checks in US dollars and major credit cards are accepted with a valid piece of identification (picture ID).
