Please note: Do not attempt to complete the form on-line. The form must be printed on individual pages (American letterhead or A-4 size), double-paged applications are not accepted. When downloading the application, please disregard the instructions on the actual passport application as special rules apply to renewal through the Embassy or the Consular Agencies in Geneva or Zurich. Please follow the instructions stated on the Embassy website.

DS-64: Statement Regarding Lost or Stolen Passport

DS-82: Application for Passport by Mail (Renewals only)

Use this form if you are eligible for a mail-in application, even if you choose to come and drop off your application at the Embassy or the U.S. Consular Agencies in Geneva or Zurich personally

DS-4085: Application for Additional Visa Pages

DS-5504: Passport Re-Application Form (for corrections within one year of issuance)
